Family Medical Quality Standards

Family Medical Center provides the highest quality of clinical care and monitors clinical performance regularly to ensure this high standard of care is maintained.

Each year, health centers report on their performance using the measures defined in the Uniform Data System (UDS) by the federal government. The UDS is a standardized reporting system that provides consistent information about health centers to allow for comparisons between centers and trends over time.

Several measures that are reported to the Health Services Resources Administration (HRSA) are monitored monthly and shared with individual providers to provide them the feedback to continually improve care include the following:

  • Colorectal Cancer screening

  • Cervical Cancer Screening

  • Diabetes A1C >8

  • Depression Screening and Follow Up

  • Hypertension control

  • Adult BMI (Weight) Nutrition and Counseling

  • Tobacco screening and cessation

  • Aspirin Use (Ischemic Vascular Disease)

  • Lipid Therapy (Cardiovascular disease)
